Welcome, New Clients!

We’re excited to have you and your pet on board with us. To make your transition into our practice easier, please complete our new client form below. Be sure to provide as much information as you can about your dog and/or cat, including their age. If you are coming from another vet, remember to bring your pet’s medical records along to your first appointment so we can integrate those records into your pet’s new file.

Completing the new client form ahead of your appointment will save you time (and paperwork) at our office, so you and your veterinarian will be able to focus on what’s really important--your pet.

What to Expect at Your First Visit

At Long Lake Animal Hospital, we pride ourselves on being flexible to accommodate our clients’ and patients’ needs. We operate by appointment and on a walk-in basis, so expect to encounter other people and pets when you arrive. If you have a small dog or cat, make sure they are in a secure and spacious pet carrier. Any dogs not in a carrier should be leashed at all times, both for their safety and the safety of other pets.

Once you walk in, a member of our front desk team will warmly welcome you and your pet and check you in.
